LPN The Mint Health Drugs: CMP Home Care – Outreach Nursing Team is a nurse-led outreach team dedicated to engaging individuals experiencing health and social inequity. The home care team manages a caseload of approximately 30 clients, operating 7 days a week, 365 days a year to provide home care services tailored to medication administration and chronic disease monitoring and follow-up. Most clients reside within housing programs, shelters, and facilities across Edmonton that are staffed by the non-profit organizations we work with, however, a small subset of individuals also reside within independent living settings (e.g. apartment complexes, houses, etc.). Hiring for the Following Hours of Operation: Every Other Weekend – Saturday and Sunday – 8:00 AM – 3:00 PM Other shifts may be offered in addition to the above based on business needs.

Key therapeutic areas include Mental Health, Substance Use Disorders, HIV, Hepatitis C, and Chronic Disease Management. Additional Details: The hours for this position will primarily fall between 07:30 AM – 2:30 to 3:30 PM. This will be a casual position for sick leaves/vacation/emergency coverage with no pre-defined schedule and the opportunity to transition into more regular hours as our program expands. Ideal for candidates who currently hold casual positions elsewhere or those who primarily work evenings at other employment sites Scope of Services: Comfort in independently assessing and dosing witnessed medications such as methadone, Suboxone®, and Kadian® Communicate clearly with patients, peers, management, and other healthcare professionals through various media using effective and appropriate communication skills.

Coordinate appointments, access to care, and social services for clients. Advise the patient to seek further medical attention where appropriate. Answer any questions requiring clinical knowledge which fall within the scope of practice as a licensed practical nurse in Alberta. Maintain confidentiality of patient information.

Provide patient health education, harm reduction, and employ principles of preventative medicine. Provided clinical care related to physical assessment, minor ailments, medication therapy, and activities of daily living where required and within the scope of practice. Follow the principles of person-centred care when providing holistic care. Provide wound care and maintain the cleanliness and stock of supplies and equipment.

Ensure accurate and timely completion of appropriate documentation, including client charting in an eMAR (electronic medication administration record). Provide leadership in the management of client care. Coordinate and communicate with a multidisciplinary team to demonstrate patient-centered care. Demonstrate willingness to learn new skills as required. — Mint Health Drugs: CMP(Community Members Pharmacy) was opened in 2012 with the mission of serving our community members within Edmonton’s vulnerable population sector.

The LPN is expected to meet and exceed the standards of patient centered care as services are performed within the scope of practice for an LPN in Alberta. There is a focus on ensuring service and clinical excellence. Responsibilities Patient Care and Service Excellence Respond to patient referrals within internal and external timelines Complete a comprehensive care assessment in a timely manner, utilizing tools, forms and processes as aligned with the Mint Health Drugs policies and procedures manual Communicate clearly with patients, peers, management, and other healthcare professionals through a variety of media using effective and appropriate communication skills Advise the patient to seek further medical attention where appropriate Answer any questions requiring clinical knowledge which fall within the scope of practice as a Licensed Practical Nurse in Alberta Maintain confidentiality of patient information.

Educate patients on technical aspects of appropriate use of home monitoring. Participate actively in pharmacy services, including written policies and procedures on efficient clinical systems in various settings, including but not limited to pharmacies, institutions, facilities, group homes, and shelters. Follow the principles of patient-centred care when delivering care Maintain the cleanliness and a stock of supplies and equipment Ensure patient and care provider safety by accurately assessing risk using tools provided Ensure that care plans meet individual patient’s needs Ensure all internal and external reporting meets or exceeds all standards and expectations of Mint Health Drugs Ensure accurate and timely completion of appropriate documentation, including client charting Actively identify potential risks with internal stakeholders and work collaboratively to mitigate or manage risks, especially those associated with clinical care Leadership Provide leadership in the management of client care Coordinate and communicate with a multidisciplinary team to demonstrate patient-centered care Participate in potential interdisciplinary Mint Health Drugs projects Be an ambassador of Mint Health Drugs Involve oneself in possible research, community education, and collaborative initiatives Demonstrate willingness to learn new skills as required Education and Experience: Active registration and valid practice permit with the College of Practical Nurses of Alberta (CLPNA) Graduate of an accredited Licensed Practical Nursing program 1 year experience practicing as an LPN, home health service delivery preferred 1 year experience practicing as an LPN, home health service delivery preferred Active Alberta Class 5 Driver’s License, Clean 3 Year Drivers Abstract Police Information Check with Vulnerable Sector. Certifications (or willingness to obtain): Standard First Aid and CPR.

Training in Trauma Informed Care, Cultural Sensitivity, Stigma, and/or Harm Reduction is preferred.
