Responsibilities: Responsible for leading the implementation the Senior Friendly Care Framework with focus on long term sustainability after temporary position has ended ensuring geriatric clinical capacity across the continuum aligned with best practices.

This work would align with ALC Leading Practices to Prevent Hospitalization and Extended Stays of Older Adults Self Assessment from Ontario Health Work collaboratively with health care providers and community support services in assessing and developing/building on current Senior Friendly initiatives at JBH Assess gaps and learning needs in geriatric care and develop and implement a plan to address geriatric knowledge and capacity Coach the geriatric approach to patient care including patient engagement and transition planning Track quality indicators related to deliverables, and develop standardized reporting metrics to monitor and report activities to ensure sustainability Facilitate and monitor the use of effective clinical tools, referral and communication processes Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement related to senior friendly initiatives Determine and align improvement projects with JBH’s Strategic Plan; monitor and adjust to achieve goal outcomes Perform other duties as required Qualifications: Minimum of a four (4) year Bachelor’s Degree in a health related field, from an accredited university Master’s degree in a health-related field is required Current Certificate of Registration in good standing as a Health Professional with an Ontario Professional Association or regulatory college Advanced training in specialized geriatrics is required Minimum of five (5) years’ experience working in specialized geriatrics across the care continuum including previous experience implementing the Senior Friendly Care Framework Experience in process improvement, change management and project planning is required. Preference will be given to those with their PMP certification Demonstrated capacity building and facilitation skills Demonstrated knowledge in patient and family-centered approaches in geriatric care planning and treatment Demonstrated training, experience or utilization of lean methodology for process improvement Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems Demonstrated superior interpersonal and communication skills, both written and verbal Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required To be considered for this exciting opportunity please visit the JBH Careers page to apply to posting 162613-67.
